The part that matters

A clinician reads it — and doesn't give Marcus what he asked for.

He asked for the full dose he'd seen online. Reviewing his history, the clinician recommends starting materially lower and explains why, in writing. He's approved, but not for what he requested.

"At your starting point, the higher dose mostly buys you side effects. We'll start lower, watch how you respond, and adjust. If you need more later, you'll get more."

It ships — and keeps changing.

Week four he says appetite shifted more than expected; the dose is adjusted. The adjustments are the product— not a fixed monthly package that ignores how he's actually doing.
