Hormone Changes in Women After 35: What's Normal, What's Not, and What to Do

Perimenopause can begin a decade before menopause. The symptoms — fatigue, irregular cycles, brain fog, disrupted sleep — are often dismissed. Here's the biology and what actually helps.

A 38-year-old woman goes to her doctor. She's exhausted despite sleeping 8 hours. Her periods have become irregular. She's gained weight around her midsection despite not changing her diet. Her brain fog has gotten worse. She can't sleep through the night the way she used to.

Her bloodwork comes back within normal ranges. She's told she's fine. She's told it's stress. She's told she's "a little young" for hormones to be the issue.

She's not fine, and she's not too young. She's in perimenopause — and it often begins in the mid-30s.

The Timeline Most Women Aren't Told About

Most women are told that menopause happens around age 51. What they're not told is that the hormonal transition leading up to menopause — perimenopause — can begin 8–12 years earlier. The hormonal changes that cause symptoms start in the mid-to-late 30s for many women, long before periods stop or become dramatically irregular.

The reason this window is so often missed: the standard lab panel isn't sensitive enough to catch early perimenopause, and the symptoms overlap with a dozen other conditions. FSH (follicle-stimulating hormone) and estradiol — the hormones typically used to assess menopausal status — can appear normal even when the hormonal environment is shifting significantly.

What's Actually Happening

Ovarian reserve — the number of eggs available — declines with age. As egg quality and quantity decrease, the ovaries begin producing less progesterone and, eventually, less estrogen. Progesterone typically declines first.

Progesterone is a calming, sleep-promoting hormone. It counterbalances estrogen. When progesterone drops while estrogen remains relatively stable, you get what's called "estrogen dominance" — a relative excess of estrogen to progesterone. This produces:

  • Sleep disruption, particularly waking in the early morning hours (3–5 AM)
  • Anxiety, irritability, and mood swings
  • Heavier or more irregular periods
  • Breast tenderness
  • Weight gain, particularly around the hips and abdomen
  • Worse PMS than you had in your 20s

The Estrogen Fluctuation Phase

Later in perimenopause, estrogen itself becomes erratic — swinging from high to low unpredictably rather than following the smooth monthly cycle you had in your 20s. This estrogen volatility is what causes many of the classic symptoms: hot flashes, night sweats, brain fog, vaginal dryness, joint aches, and mood instability.

Important distinction

Perimenopause symptoms aren't caused by "low estrogen" — they're often caused by estrogen volatility. This is why some women feel worse on estrogen-only therapy if their levels were already fluctuating high.

Getting the Right Workup

A single FSH and estradiol test on day 3 of your cycle isn't sufficient to fully assess your hormonal status, especially in early perimenopause. A more complete workup includes:

  • Estradiol (day 3 and day 21 of cycle, to capture both phases)
  • Progesterone (day 21 of cycle — post-ovulation)
  • FSH and LH (day 3)
  • Total and free testosterone (often low in women in their 30s-40s)
  • DHEA-S (adrenal androgen precursor)
  • Thyroid panel (hypothyroidism mimics many perimenopausal symptoms)
  • Fasting insulin (insulin resistance worsens dramatically in perimenopause)

What Actually Helps

The evidence base for perimenopausal symptom management has expanded substantially in the past decade. Options, depending on symptom pattern and lab results:

Progesterone (Bioidentical)

For women in early perimenopause whose primary issue is progesterone decline, bioidentical progesterone (oral micronized progesterone like Prometrium, or topical cream) can meaningfully improve sleep, mood, and cycle regularity. It's distinct from synthetic progestins, which have a different risk profile.

Estrogen Therapy

For women with significant estrogen-related symptoms, transdermal estradiol (patch, gel, or spray) avoids first-pass liver metabolism and has a more favorable risk profile than oral estrogen. The Women's Health Initiative data that scared a generation of women and doctors away from hormone therapy used oral conjugated equine estrogen — not the bioidentical transdermal estradiol studied in more recent trials.

Testosterone

Women produce testosterone in their ovaries and adrenal glands, and levels decline with age and during perimenopause. Low testosterone in women causes fatigue, reduced libido, difficulty with motivation, and muscle loss. Low-dose testosterone therapy (off-label in the US, but supported by evidence) can address these symptoms.

Lifestyle Foundations

Resistance training becomes more important in perimenopause, not less — it preserves muscle mass, improves insulin sensitivity (which deteriorates significantly in this period), supports bone density, and improves mood. Prioritizing sleep, managing stress, and reducing alcohol intake all have outsized effects during the perimenopausal transition.
