What Actually Happens in Your Body When You Stop Eating for 16 Hours

Time-restricted eating is the most widely practiced form of intermittent fasting. The benefits are real — but so are the common mistakes. Here's the hour-by-hour biology and how to do it right.

Sixteen-hour fasting — eating within an 8-hour window, fasting for 16 — has become the most widely practiced form of intermittent fasting. It's accessible, doesn't require calorie counting, and fits into most schedules. It also has a meaningful evidence base.

But most of the conversation around it focuses on weight loss and skips the actual biology. Let's go through what's happening inside your body hour by hour.

Hours 0–4: Digestion and Glucose Metabolism

Your last meal is being digested and absorbed. Blood glucose rises, triggering insulin secretion. Insulin signals your cells to take up glucose for energy and stores excess glucose as glycogen in the liver and muscles. Nothing unusual is happening — this is the fed state.

Hours 4–8: Post-Absorptive State

Digestion is complete. Blood glucose has returned to fasting levels. Insulin has dropped. Your body is now running on glucose from liver glycogen stores — not from a meal. This is where the transition from fed to fasted begins. Most people spending time here are sleeping.

Hours 8–12: Glycogen Depletion Begins

Liver glycogen stores begin to deplete. As glucose availability decreases, glucagon (the counter-hormone to insulin) rises, signaling the liver to begin producing glucose from stored glycogen. Insulin continues to drop. Fat cells begin releasing fatty acids for energy use. This is where the metabolic shift really begins.

Hours 12–16: The Fasted State

This is where time-restricted eating distinguishes itself from simply "not snacking." Liver glycogen is nearly depleted. Fat oxidation has increased substantially. The liver begins producing ketone bodies from fatty acids — alternative fuel sources that the brain can use in place of glucose. Ketones are present in low but measurable concentrations.

  • Insulin is at its lowest point of the day — cells are maximally insulin sensitive when you break the fast
  • Growth hormone is elevated — GH peaks during fasting and sleep, supporting tissue repair and fat mobilization
  • Autophagy is accelerating — the cellular cleanup process that clears damaged proteins and organelles
  • Inflammatory markers are declining — fasting has acute anti-inflammatory effects
  • BDNF (brain-derived neurotrophic factor) may be elevated — potentially explaining the mental clarity many people report

What the Research Shows

A 2019 study in Cell Metabolism found that time-restricted eating (10-hour window) over 12 weeks improved insulin sensitivity, reduced blood pressure, and reduced visceral fat — without explicit caloric restriction. A 2020 New England Journal of Medicine review by Mattson et al. summarized the mechanisms: metabolic switching from glucose to fatty acid oxidation, improvements in multiple cardiometabolic risk factors, and potential neurological benefits.

Important caveat

Most of the benefits of time-restricted eating appear to be independent of caloric restriction — meaning they occur even when total food intake doesn't decrease. But when people also reduce calories while time-restricting, the benefits compound.

The Most Common Mistakes

Eating too late in the evening

Circadian biology matters. The same calories eaten at 7 PM vs. 9 PM produce different metabolic effects. A 2019 study found that eating within 2 hours of bedtime significantly impaired glucose tolerance. The most metabolically beneficial windows are earlier in the day — 8 AM to 4 PM or 10 AM to 6 PM. Most people time-restrict by skipping breakfast, which means their eating window often runs 12 PM to 8 PM. This isn't wrong, but it captures less of the circadian benefit than an earlier window.

Not eating enough protein when eating

If you compress your eating window and don't prioritize protein, you will lose muscle mass. Protein intake during the eating window needs to be at least 1.6 g per kg of body weight — possibly higher if you're active. Spreading protein across 2–3 meals within the window is better than concentrating it in one meal.

Breaking the fast with sugar

After 16 hours of fasting, your cells are maximally insulin sensitive — which means a high-glycemic first meal will produce a large glucose spike and significant insulin release. Breaking the fast with protein and fat (eggs, avocado, nuts) rather than carbohydrates (juice, cereal, bread) extends the metabolic benefits of the fasted state rather than immediately reversing them.

Who It's Not Appropriate For

Time-restricted eating is not appropriate for everyone. It's not recommended for people who are pregnant or breastfeeding, individuals with a history of eating disorders, people who are underweight or have low muscle mass, those on insulin or blood sugar medications without medical supervision, or adolescents and children. If any of these apply, consult a clinician before attempting any fasting protocol.

The simple version

Stop eating 3 hours before bed. Don't eat again until you've been awake for 3–4 hours. Prioritize protein when you do eat. That's 90% of the benefit with none of the complexity.
