"Normal" on a lab report means you fall within the range of the average American — not that you're healthy. Here's why reference ranges are set the way they are, and how to read your own labs.
You've had bloodwork done. The report comes back with a column of numbers and a column that says "Normal" next to each one. Your doctor glances at it, says everything looks fine, and you're done. Until a few years later, when something isn't fine.
The problem isn't that your doctor is careless. The problem is what "normal" actually means — and most patients have no idea.
How Lab Reference Ranges Are Set
Reference ranges are calculated by testing a large sample of the general population and taking the middle 95% of values. If your number falls within that middle 95%, you get the "Normal" stamp. If it's in the outer 5%, you get a flag.
This sounds scientific. But it has a fundamental flaw: the general population includes millions of people who are metabolically unhealthy, sedentary, sleep-deprived, and chronically stressed. "Normal" is the range of the average American — and the average American is not in optimal health.
The math problem
Roughly 88% of Americans have at least one marker of metabolic dysfunction. When the population used to set reference ranges is largely unhealthy, "normal" and "healthy" are not the same thing.
The Fasting Glucose Example
The "normal" range for fasting glucose is 70–99 mg/dL. Prediabetes begins at 100 mg/dL. Your doctor gets the result back showing 97 mg/dL and reports it as normal.
But metabolic health researchers would flag 97 mg/dL as suboptimal. Optimal fasting glucose is 70–85 mg/dL. A result of 97 mg/dL suggests your body is working progressively harder to keep blood sugar in range — it just hasn't started losing that battle yet. By the time the number hits 100, the battle has already been going on for years.
The Testosterone Example
The "normal" range for male total testosterone spans from approximately 264 to 916 ng/dL — a range so wide it includes both men who feel exhausted and depleted and men who feel excellent. A 35-year-old man with a testosterone of 280 ng/dL is technically "normal" by this standard. But 280 ng/dL is a level typically seen in men over 70. "Normal" and "optimal" are not the same number.
The TSH Example
TSH (thyroid-stimulating hormone) controls your thyroid, which regulates virtually every cell in your body. The standard reference range is roughly 0.4–4.0 mIU/L. A TSH of 3.8 is technically "normal." But many functional medicine physicians and endocrinologists consider anything above 2.5 worth investigating if symptoms are present — fatigue, weight gain, brain fog, cold intolerance, hair thinning. Patients with a TSH of 3.8 who are told "your thyroid is normal" are often not treated, when treatment might meaningfully improve their quality of life.
What to Ask Instead
When you get bloodwork back, the right question isn't "am I normal?" It's: "where am I in the range, and which direction am I trending?" A result at 30% of range is very different from a result at 90% of range, even if both are "normal."
- Ask for your actual numbers, not just the flag status
- Ask where in the normal range your results fall — top, middle, or bottom
- Compare to prior results to see if you're trending toward or away from the edge
- Look up optimal ranges from functional medicine or longevity medicine sources, not just disease-threshold ranges
- If you have symptoms, don't accept "normal" as the end of the conversation
Markers Where "Normal" Is Particularly Misleading
- Fasting glucose: Optimal is 70–85; "normal" goes to 99
- Total testosterone: Range is too wide to be clinically meaningful without free T and SHBG
- TSH: Anything above 2.5 with symptoms warrants further evaluation
- Ferritin: "Normal" extends down to 12 ng/mL; optimal is 50–100 ng/mL for most adults
- Vitamin D: "Normal" is above 20 ng/mL; most researchers consider 40–60 ng/mL optimal
- Total cholesterol: High total can be misleading without particle size and ApoB data
The bottom line
You are not a statistic. Lab ranges describe populations, not optimal individual health. Learn to read your own numbers — not just the Normal column.
